Call 507-227-2757 to set up your appointment. You will be asked to provide a cell phone number and email address. You will then indicate which is your preferred method for us to contact you. You will then receive pre-visit forms to fill out prior to your appointment. Completing these forms before your appointment will help with the efficiency of your visit and allow you more time to express any concerns you may have.
